No. There's no evidence supporting it and he has no reason to. Freeza's lower forms only exist as a means of controlling his power, something Cold seems to have no issue with.

Yes. If he wasn't, he'd have used it rather than resorting to trickery.

Cold has one form or is in his final form. I actually hypothesize that other forms would weaken him or do nothing, which is why he knew he waa screwed.
