I think my theory below makes the most sense.
Zenkai was introduced as a substansial boost Saiyans get when they survive a near death battle wound. Later, though, we see them getting them even from minor injuries and they're way too large. My explanation is those boosts were not zenkais.
The only times we saw Saiyans being really beaten to an inch of their life were...
-Goku/Vegeta after their fight
-Vegeta after Zarbon
Those were really beatings that could explain a zenkai and the boosts were like 1.2x-1.33x (i guess we can
deduce all true zenkais are around that range and not insanely large). After that...
-Vegeta gets beaten up by Recoome. He's injured, but not near death; he can even stand up. I guess he got
a small increase because he fought to his limits, but not a zenkai. That small increase, though, was enough
to break the wall of what a normal Saiyan is supposed to be capable of. As a result, his power started
continuously increasing for the next few hours, slowly evolving him into a Super Saiyan (his heart is what
hindered the final transformation when he fought Freeza).
-The same stuff goes for Goku. His body (when it was possessed by Ginyu) only received a few hits from
Vegeta; he didn't really look near death. I guess we can say the slow proccess of the steady power
increase had already began when he landed on Namek, or maybe it started after the hits from Vegeta,
which could count as a fight to the limits which gives a small increase.
-As for Gohan, his power is the most tricky. Unless Freeza's foot snapped his neck or something, i don't see
how he could be considered near death when Dende healed him. Maybe the increase came from Guru's
boost, which hadn't ended yet. Or maybe his rage became greater the second time he attacked Freeza.
