How does no one inside the show notice Yugi losing and gaining 2 feet every 10 minutes?

In-universe he isn't supposed to grow. Both personalities actually have the same height in the manga, but grow steadily taller as the series progresses.

Yeah, I was about to say, there's no height difference in the manga. However, the 4kids dub of the anime does point out the difference a few times in the Duelist Kingdom arc.

Well in the first episode of the anime after Yugi transformed Kaiba did have a "What the fuck?" reaction to Yami Yugi showing up. Otherwise I think the difference are put in there for the audience to be able to tell the difference between the two. Otherwise they seem the same aside from noticing Yami Yugi's drastically different personality from Yugi. The height thing I would credit to art inconsistency thing. Yugi's height changes I dunno know how many times

In Battle City Yami Yugi's like 2 feet taller or close to it.

In Yugi & Yami Yugi's duel they're the same height (or Yugi looks taller due to his huge ass eyes)

After the duel Yami Yugi looks taller again
