Do you think AT had only planned up such state amidst the story (as Goku seems to ignore it entirely when saying he can't defeat Fat Boo) or he had it already planned from the beginning, but decided to keep it unmentioned to build up the drama?

The fact that Goku visibly sweat drops and then goes emotionless when Goten and Trunks angrily ask why he couldn't stop Boo (and he replies with a dragged out "I wish I really was the strongest, but I really really am pathetically weak" or the likes) suggests Akira Toriyama was already planning it.

While indeed it came out of nowhere with no story behind it but who knows? It can work either way.

He definitely didn't plan it, given Galu makes it apparent he can do nothing to Boo before asspulling this form. The form contributing nothing to the plot makes it pretty apparent how little role it played in Toriyama's plans beyond momentary spectacle.

If it was a spontaneous idea, AT used it at the wrong time. He should have saved it for Kid Buu Saga. If Goku goes SSJ3 against Kid Buu for the first time...
-Nothing changes in the plot.
-SSJ3 is given a chance to shine.
-Kid Buu can be treated as the strongest Buu, which makes more sense.

SSJ3 sucked. Did nothing and looked stupid on-top of it all.

That would be even worse, since Goku would be holding back for absolutely no reason than to extend the plot and if he didn't destroy Boo with it, the form would still be a pointless addition. It wouldn't make Pure Boo being the strongest make sense, unless you also remove Evil Boo freaking out at the idea of Vegeta ripping Fatty from him as well as Gotenks Boo referring to himself as the strongest Majin. Even if you have Goku just achieving SS3 in that fight, it would come off as just a contrived deus ex machina.

Which as I said, doesn't make an asspulled power up or having a contrived way to make Goku the hero again any better due to Pure Boo > Other Boos still not making any sense and SS3 just becoming more of the same, and even more worthless if it doesn't kill Boo.

It'd probably be even worse than that, considering at least PSSB had some build up from SSB's stamina problems to excuse it within the plot.

SS3 is just pointless fanservice, regardless of how it's used.
