1. Piccolo revealing about Dragon Balls to Raditz therefore Vegeta and Freeza went on to find it which caused a lot of disasters.

2. Goku and Vegeta on not agreeing about Bulma's idea about finding Gero to prevent the Androids from the beginning.

3. Vegeta by letting himself to be controlled by Babidi so he can matched Goku's power and fight him hence Boo is released.

4. Trunks going back to the main timeline and gave Goku an antidote to prevent him from virus therefore with Goku being alive then Zamasu stealing Goku's body which results the Zero Mortal plan.

5. Goku starting the tournament of power only because he wants to fight the strongest fighters in the multiverses.

I don't understand what you mean. I don't think any of these "damaged the series."

If you're talking about stupidity, I think Goku not killing Fat Boo when he had the chance was pretty stupid.

Goku not going with Bulma's plan to take out Gero early. His childhood self had little mercy for those who actively endangered his friends' lives. Even with his weak reasoning of Gero having not done anything yet, there's still nothing wrong with them cornering Gero and either destroying his equipment or taking him captive. This could be seen as the birth of the Idiot Goku we see primarily in GT and Super, as well as with several big mistakes in the Cell/Boo Arcs, so this is vicariously the start of the problem.

If we're talking from an in-universe sense, then Piccolo telling Raditz about the DBs purely for the fact we can infer from Nappa's permanent death that the population of East City weren't wished back to life with Popo's wish in the Freeza Arc. The rest of the cases were ones in which the deaths of those involved were for the most part completely reversed. The only exception is with the Future Trunks Arc, in which the universal erasure still doesn't matter when a 100% identical timeline to that prior to Zamasu's meddling still existed.
