I think Cell could regenerate even if one cell remained.

Since we've seen Goku shifting his vital spots in Super, I'm gonna go ahead and say Cell sent it down to his ass the moment Goku unleashed the KMHMH.

But ignoring Super, it certainly seems like a plot-hole to me. I'm not sure how else you can take the scene.

Even with Super it's still a plothole. Goku could only do that with Ultra Instinct.
