As seen in the Majin Buu Arc, since the SSJ3 has a lot of tension, the Kamehameha does not have much amplification.
I think Goku SSJ3 (Kamehameha) > Hirudegarn > Goku SSJ3 (M13).
In any case, Hirudegarn would survive and SSJ3 Goku would be exhausted.
