They probably think it's the God Hand's work and Guts starts screaming "GRIIIIIFIIIIIIIITH!!!" He goes on a rampage, killing anyone who looks slightly strange (eg. the anthropomorphic animals). What happens next purely depends on what era he appears in.

He immediately one shots Botamo.

That makes him gutless.
