Yeah, he does look worse for wear than Vegeta and Trunks. He seems to be hunched over from exhaustion and is clearly in pain, whereas Vegeta and Trunks are just standing there.

I don't think it's quite the same. Vegeta and Trunks were straight looking ahead into what surprised them focusing on the thing besides them, while Piccolo was doubling over a little and cleaning his face, showing more visibly affected by the battle. It's not a big difference for sure, but I wouldn't say it's hardly any difference either. When Piccolo was first shown, he also had a more intense bleeding and darker bruises. I think Piccolo is relevant, but still in a lower tier than Vegeta and Trunks for sure, perhaps just like in the androids saga.

In the non-title page, Piccolo is slightly more marked than Trunks and than what we can see of Vegeta's sideface, but nothing major. In the title page, Vegeta's mark looks darker than in the first page, so there's some inconsistency in the shading of damage. The most obvious difference is Piccolo in the title page looks more tired (posture, mouth open). But I wouldn't say it's enough of a difference to say he had a harder time with the Cell Jrs - it could be that he's not more tired, but just happens to be in a battle stance for the cover, whereas Trunks and Vegeta are being shown in their "shocked about Gohan" stances.

Doesn't seem so much like a battle stance to me, sound more like recovering from the intensity of the fight a little, while Vegeta and Trunks seem more composed since they are reacting to Gohan. I think the fight was more relevant to Piccolo in a way he could barely keep up, which is why reacting to Gohan wasn't the first priority but getting a breath imo, while Vegeta and Trunks were fighting evenly and then more in tune with all the surroundings.

Yeah I think the things with the damage face is a little inconsistent, but at least in that panel, Piccolo's bleeding seemed a little more intense and his bruises a little darker.

But that's just a title page. In the actual page, nothing suggests Piccolo reacts later and his posture is leaning back and not in a (possibly) tired forward posture.

In this battle stance he's looking forward and focused. There he was cleaning his face and with his mouth open a little, so it was more about being tired imo. True, he wasn't doubling over later, but I don't think that side page is fully consistent with the way they were in the actual pages 100%, more like a recap on their conditions overall post battle, without literally translating to every detail imo.
