I think he needed it once when Bulma was having a bath? That right? He never needed it mid-fight, like in Gintama.

No. He was offering to wash her back because she didn't have a tail.

I remember only a couple is times as a kid. Once when he was interacting with Oolong for the first time, and... I don't know... Some other time?

I vaguely remember a scene with either Goku or Gohan needing to pee and accidentally peeing onto someone’s head a floor below them. I think it was Gohan in Dead Zone though, so it doesn’t count.

Still, I’m pretty sure we must have seen Goku needing to pee in the early days of the series. Not so much in Z though, even with anime filler.
