Why was Android 16 deemed a failure?

I don't see how it's any different than releasing 17 and 18, who had already proven they would not follow his orders.

I'm sorry man, but I just can't buy the logic of Gero not even being willing to try 16. He clearly is able to control 16 in the same way as 17 or 18, so turning him off shouldn't be a problem if he decided to turn on Gero.

That's correct. It was exactly like with #17 and #18 - defective creations he only thought of using as an absolute last resort and was still hesitant about doing so. We already saw that the twins could get the better of Gero and disarm him of the remote. Against someone who is far stronger and logically far faster than them, a remote with a limited range as his only safeguard isn't exactly a foolproof plan.

I think I might have seem this somewhere, but the failure part was just a bluff on Gero’s part. 16 was modeled after his son, and he didn’t want to lose him in battle.

That's more of an assumption based on the revelation tbh. The only work that's weighed in on Gebo (Gero's son) being a factor would be DBZ Kakarot, in which #16 states he was a failure in that Gebo was an obedient and efficient soldier whereas #16 is far too kind hearted to live up to what Gero expected.
