Yamato might be the stronger fighter but Marco's regen would be a huge problem. This man went up against Kizaru after all. I'd give it to Marco.

How much of a difference does having advanced Haoshoku make? Does it even affect Marco?

A big difference. It would just mean that she'd fare even worse without it.

Big Mom also didn't want to fight Marco in the state she was in. I think that implies that Marco at full stamina would be able to stall a Yonko for at least as long as Yamato was capable of doing. I think he wins.
