Is it achieved through rage, training, or both? I tend to think it's usually done through constant training, and that Gohan's a rare case where his ability to bring out his dormant power through rage allowed him to achieve it.

The simplest answer is "it depends". All statements about how Goku and Vegeta achieved the form only mention their intense training, whereas rage was clearly the catalyst for Gohan and Trunks. Perhaps this is mainly due to the genetic differences between full-blooded Saiyans and hybrids, or that the fathers having far greater fighting experience allowed them to reach that point without an intense emotional response.

I'd highly doubt it's based on rage aside from Gohan. There was no reason for Goku or Vegeta to have any rage during the 7 year gap.

I suppose so, though I can't see Goku or Vegeta becoming that enraged through visualization. Goku was at peace with everything when he died and Vegeta had lost the motivation to fight by the end of the Cell arc. I can;t see either of them becoming that enraged with the mindset they were in. Gohan was also a child and it was easier for him to tap into those emotions at the time. We even see that he was unable to tap into his rage after he matured into a teenager. I don't think Goku or Vegeta would be able to either without a reason for it.

I can see your case with Goku, but Vegeta suffered the ultimate humiliation after the events of the Cell arc. He even went out of his way to become Majin after seeing that all his hard work turned to nothing in the face of Goku's power. It's basically the same way he turned SSJ1.

Vegeta had come to terms with his situation after the Cell Game though with him admitting his fault to Gohan and even declaring he'd never fight again after seeing the sacrifice of Goku, something he seemingly would've held true to if not for Goku's one day return as he seemed to have no interest in training beyond bettering himself in those times. With his clearly different demeanour after Trunks' death, I highly doubt he was being fuelled by humiliation when training as settling the score with Goku was seemingly an impossibility in his eyes and it's not as though he moved onto directing Gohan as his goal.

I view Gohan's SSJ2 as something unique to him, which is why it makes him stronger than Adult SSJ2s even after he slacks off for 7 years.
